Salary range: £37,293 - £40,086 p.a. inc.

The Post

This is an exciting post and provides the opportunity to work across health and social care and advance your practice enabling you to gain an in-depth understanding of integrated working at a time when this is a key national driver.

Employed by the Local Authority you will be the subject expert and point of advice for social work staff, and will proactively work with the CHC Nursing team to undertake assessments and review of need for those identified as potentially eligible for full or part CHC funding.

The Person

Are you driven by working holistically and in partnership to ensure the best outcomes for individuals?

Do you have a sound working knowledge of the National Framework for NHS Continuing Health Care and relevant assessment tools?

We are looking for an exceptional social worker who is committed to ensuring individuals have access to what they are entitled to; and can work in partnership with CCG colleagues to ensure delivery of fair and equitable health and social care support, founded on assessed and evidence based need.

The Post

The post acts as the CHC lead for Adult Social Care and works closely with the CCG to support the timely and effective assessment and review of CHC, complex care and joint funded support. Providing expert advice to both health and social care colleagues and ensuring effective monitoring and reporting of CHC arrangements and ensuring citizens have equitable access to.

The Person

You must be an HCPC registered social worker with an extensive working knowledge of the Continuing Health Care Framework and Care Act as well as key legal judgements. Have the ability to work autonomously and in partnership and the ability to provide person centred and evidence based assessments to aid sound decision making.
